Description:

We are currently seeking a full-time Population Health Consultant (PHC) to join our team in our Salt Lake City, UT office. This position will support regional offices (can include UT, AZ, CO, NV, ID, TX) and may require minimal travel.

The Population Health Consultant serves as a strategic advisor to employer groups, partnering with clients to assess, develop, implement, and evaluate population health and wellness strategies that improve workforce health outcomes and support business objectives. Through data analysis, health education, and evidence-based recommendations, this role guides clients in optimizing wellness programs, enhancing employee engagement, managing healthcare costs, and addressing population health risks. Success in this role requires strong consultative relationships, effective communication, and collaboration with clients, carriers, vendors, and internal teams to drive measurable improvements in employee well-being, productivity, and organizational performance.

Essential Duties and Responsibilities:

• Develop and implement customized population health strategies and wellness initiatives aligned with clients’ workforce health goals, organizational priorities, and business objectives, while establishing measurable outcomes, monitoring performance, and driving continuous improvement in population health results.

• Analyze healthcare claims, biometric screening, engagement, and demographic data to identify population health risks, emerging trends, and opportunities to improve health outcomes and manage healthcare costs.

• Evaluate and optimize employer-sponsored wellness and population health programs through outcomes analysis, performance measurement, and continuous improvement strategies.

• Provide consultative, data-driven recommendations and strategic guidance to clients on population health management, employee well-being, preventive care, chronic condition management, and healthcare consumerism.

• Design targeted health, wellness, and benefits communications that promote employee engagement, support informed healthcare decisions, and increase awareness of available programs and resources.

• Develop and deliver health education initiatives through seminars, webinars, workshops, health fairs, digital content, and other engagement platforms to support behavior change and health improvement.

• Oversee wellness program evaluations, employee surveys, and engagement assessments, translating findings into actionable recommendations that enhance program effectiveness and participant outcomes.

• Present population health strategies, insights, outcomes, and recommendations to employer groups, business partners, brokers, and prospective clients to strengthen client relationships and support business development efforts.

• Advise clients on wellness program design, implementation, and incentive strategies while ensuring compliance with HIPAA, ACA, EEOC guidance, federal wellness regulations, and other applicable requirements. Education, Licenses and Certifications:

• The minimum level of education recommended for candidates in this position is a

bachelor’s degree in the health sciences or a related field.

• Certified Health Education Specialist (CHES) preferred.

• Additional Wellness, Fitness, and/or Coaching certifications desired.

Skills and Knowledge:

• 3+ years’ experience in population health consulting, wellness program management, or

commensurate experience.

• Professional written and verbal communication skills.

• Exceptional interpersonal skills.

• Strong business acumen.

• Human resources, employee benefits, and insurance experience preferred, but not required.

• Account management experience recommended.

Working Conditions / Essential Functions:

• This position enjoys the benefits and climate control of a quiet office environment. Employees are provided with ergonomically sound workspaces. Work is accomplished at a computer terminal either sitting or standing. Employees must access, input and retrieve information from the computer system. Comfort and efficiency are considered in employee workspaces. Daily work includes computer interfacing, data entry, communications (electronic and physical), movement throughout the office, attending meetings, etc.
